I would just have some JS control the form, and update the list div (let's make it easy) on status == 200, and then clear the form, ready for another note.
On the server side, your #create method is pretty much the same. Just make sure you return a status code that's recognizable from the JS side. I'm not sure wether you can have methods react differently on POST from JS tho. Maybe some Merb wizard will chime in and tell us.
