If you had the head off and cleaned it up (and the pistons tops) it will clatter something horrible and run badly until you get some soot built up on the head and piston crowns, and all the air out of the injection lines -- takes a few miles.

Both of our 603s did this when the head was replaced. Sounds terrible, and I got lots of white smoke for a while on mine after startup, but once it has a few miles on it it will smooth out.

Smoky rough starts are pretty much normal after servicing the injection system.

I'm more concerned about an unbalanced engine, you REALLY don't want to toss a rod.
