very good accelleration after the rpm's go up, great mileage, and this problem 
happens mostly after the car is well warmed up...

That is typical of a car that requires the ALDA (pneumatic fuel
enrichment device) to be adjusted. Usually the screw needs to be turned
counterclockwise 1 to 1 1/2 turns. If the engine smokes excessively,
you've turned it too far.
