Assuming you have taken care that IE window is opened after QTP and
web addin is selected, if still this issue persists, follow the steps
below to resolve the issue.:

1. Login as an admin account.
2. Run these steps:
regsvr32 -u c:\WINDOWS\system32\BHOManager.dll
regsvr32 c:\WINDOWS\system32\BHOManager.dll
3. Reboot the machine.
