looks like you are not adding Web-addin. Not sure whether that can be 
un-checked from the list of Addins.
Please do the following :
1. go to QTP toolbar and select Tools > Options
2. from general options on the left side of the menu, select the option 
'Show Addin manager on startup"
3. Now close and restart the QTP
4. See whether Web-addin is selected from the addin manager. If not then 
select and rexecute the code.
 
hope this will solve your issue.
