nhd2osm generates non-unique OSM id's. It apparently start at -1 going down
for every separate export.
When you try to combine exports and there is a way referencing a node with
an id existing in the previously imported file, it will use that one,
screwing up everything.

I don't see a way to solve this at Merkaartor side but, as I said, the shp
should import fine so there shouldn't be a need fiddling with nhd2osm.
