What "Run as a service" checkbox? I'm using 22.8, and there's only "Options -> Start at Bootup" in that version. First ensure that you have 22.8 installed, and not some other version. Ensure that you have "start at bootup" enabled. Go to Control Panel -> Administrative Tools -> Services Ensure that "Prime95 Service" is set to "Automatic" and "LocalSystem", and that it says "Started".
