On a related note - this really DID happen to My wife.... Several years back her medical insurance was "(something) Of Alaska." She was at a Dr.'s office and When the desk clerk asked for her insurance, she gave Her her card. The desk clerk responded, "Oh, I'm sorry, We don't take insurance from foreign countries."
