I've configured MH/nmh to define both headers.  Here is what
I have in my replcomps file:

%<{date}In-reply-to: %<{message-id}%{message-id}%>\n%>\
%<{message-id}References: %<{references}%(void{references})%(trim)%(putstr) %>
            %(void{message-id})%(trim)%(putstr)\n%>\

In-reply-to only contains the message-id to be more conforming with
RFC 2822 and some MUAs cannot parse in-reply-to that well.
