I think that somewhere around 15 or 15.2 AFR is best efficiency.  14.7 
is good for emissions.
Wider throttle openings do help with efficiency because the wider 
throttle opening cuts pumping losses - less vac sucking against a closed 
throttle plate.

You are correct about driving technique, though.  I went from 17mpg to 
22pmg in my truck by carefully adjusting the loose nut between the seat 
and steering wheel. :)
