Oh, nobody reads README :) Please, read our README file, especially sections about libv4l. According to it you should install libv4l and run mplayer as follows:

LD_PRELOAD=/usr/lib/libv4l/v4l2convert.so mplayer tv:// -tv \
      driver=v4l2:width=640:height=480:fps=25:device=/dev/video0 -vo xv

You need to use the 64-bit version of libv4l with the 64-bit version of mplayer. Some new distros have mplayer linked to libv4l by default.
