Given that the only place class="vcard" can go is the <tr>, where can I
put class="adr"?

If you have to stick to using tables (ugh), you can nest them together.

I don't - but then, it's not my website.

Then again, what's wrong with tables fir tabular data?

<tr class="hcard">
<td class="fn org"><a href="...">Visitor Center</a></td>
<td><table><tr class="adr">
 <td><img alt="" src="flag.png" />
     <a href="..." class="country-name">Nepal</a></td>
 <td><span class="locality">Okhaldhunga (40 km)
     <a href="...">map</a></td>
</tr></table></td>
</tr>

I'm trying to advise someone about marking up an existing table; uFs are not supposed to require the re-engineering of valid HTML.
