It's been a while for me but this looks like a failue while loading a
dynamic library, probably an DSO. Which is weird, because frankly I'd
expect the problem to be related to midgard, which isn't DSO loaded in the
MP. If you comment out the adding & loading of mod_midgard & php3, does it
still fail?
