Curses! This looks exactly like the FT problem I thought I solved. Only
you don't have FTs enabled so this is doubly troubling to me.

FWIW, I've been hitting it with ab (that is apache benchmark, not
Alexander :) pretty heavily without problems. This, of course, doesn't
help you any. If you want to see if the problems go away whitout the FT
code, do a clean fetch of mod and php with
$ cvs -z3 get -D '5 days ago' mod php

This I would love to know about as well. gdb is no help at all, even with
a statically compiled mod_midgard & php and running with -X. Malloc
debuggers turn out to be equally useless.

Syslog is your friend, I'm afraid. Does anyone know if there's a way to
dump a stacktrace in a running prgram by some function call?
