REPRO
1. check hidden field for trained clients
2. create new client
3. verify trained checkbox is hidden
EXPECTED BEHAVIOR:
1. trained on date is also hidden
ACTUAL BEHAVIOR:
3. trained on date is visible, and if a date is entered, error message appears
saying user must check the trained checkbox, which is impossible because it is
hidden
