SurveyInstanceActionStrutsTest.testCreate is failing after revision 16809

Yesterday I and a couple of other developers experienced a failing
integration test when building Mifos after upgrading to revision
16809. However, the test passes when building the application on
Hudson, and the test passes when run by itself in my local development
environment.
The failing test is (see below for complete stack trace)
org.mifos.customers.surveys.struts.action.SurveyInstanceActionStrutsTest.testCreate()
It could be a timing or test-dependency problem, for example, if a prior
integration test leaves the database in a state that breaks this test, but I
haven't had time to explore more deeply.
