Allow creation of a Fee w/o a specified amount

GK offers insurance to its clients-- but each clients insurance premium is
different (based on age and other criteria).
They would like to be able to define a fee, ie, "insurance premium", with no
fixed amount. Then, when it is applied to a customer-- the data entry officer
would be prompted to fill in the amount.
NOTE: GK didn't realize that you could edit fees when appling them to
customers/accounts. They are going to see if this satisfies their requirements.
