Create Permission that allows user to create a new client account if previous
client account is blacklisted

Currently we allow users to create a new account for an user if a previous
account is blacklisted, instead relying on the warning (Which is generic - see
bug 2764: Error displayed for duplicate accounts should indicate the account
found is blacklisted).
We should add a permission so that only roles have the right to create new
client accounts if a previous account is blacklisted
- or -
Or make the permission at the approval level, where only specific users have
the right to approve clients that were previously blacklisted
- or -
Or some kind of special approval, either allowing the user entering the account
to continue, or as a double approval on the account.
Different options, but someway to put in some more control on overriding
blacklisted accounts.
