All criteria are fulfilled except:
- there are still old 'Attach a survey' link instead of 'Attach a Survey'
- in the drop down labeled 'Select a Question Group' the default value is
'select one' instead of 'Select One'
Tested with 2 Question Groups (with 2 sections) which contains:
- 3 Text fields
- 2 Numeric fields (one with min and max value)
- 2 Date fields
- 2 Multiple Select fields
- 1 Mandatory Single Select field
- 1 Smart Select field
