Van Mittal-Henkle resolved MIFOS-3558.

resolved in 1.6.x and head. There are 2 payment type fields on the page, one
for disbursement and one for payment (unused and greyed out for disbursement).
The disbursement logic was incorrectly using the second payment type instance
(which was null and defaulted to CASH) rather than the first payment type
instance that the user could set to VOUCHER. That logic has been corrected.
