johnwoodlock resolved MIFOS-3619.

Resolution: Fixed
Two items of fee info not yet displayed (as not in DW yet). Also a couple of
interpretation questions remain. But marking this resolved. Can put any
additional items in another issue if necessary.
Can tackle outstanding questions on all 3 reports together (i.e. another sync
up with Kay/me at some point early next week.
Report committed to git repo and put on dev sandbox.
