Remove ability to create new additional fields or surveys in Leila E

In Leila E, we want to move everyone onto QG's. There will be automatic data
migration at upgrade. However, if there are errors, the existing additional
fields and surveys are still available.
We do not want to allow anyone to create new Additional Fields or Surveys.
Additional Fields changes:
* Remove "Define new additional field" link on Admin page
* Under View Additional Fields - remove link in heading to create new - change
to "Click on a category below to view the additional fields defined for that
category"
* Remove edit links from each custom field
Surveys
* Remove links to "Define questions" and "Define new surveys"
* Under View Surveys - change text at top to "Click on a survey below to view
details"
* Under each survey - remove "Edit Survey" link on top right
* Under View Questions - change text at top to "Click on a question below to
view details"
* Under each question - remove "Edit Question" link on top right
