CSS cleanup for Change Password page

New FTL page for Change Password needs UI cleanup. See screenshots of 1.6 and
1.7.x for comparison. Specifically, these fixes need to occur:
- center the form to middle of page
- "user name" appears - shouldn't show up on page at all.
- change last label to Confirm New Password
- get all three labels to fit on single row next to edit boxes
