when activating a question group, different questions with the same question
text are not imported as unique questions

I found this by activating PPI surveys for both Romania and Vietnam in my Mifos
dev instance.
h2. Repro steps
# activate Romania question group
# activate Vietnam question group
h2. Expected results
questions table would contain all questions for both surveys
h2. Actual results
a row with nickname ppi_vietnam_2009_survey_date does not show up in the
questions table
h2. Additional information
* If you only have one active survey, this bug won't matter
* You'll run into problems if, say, you have "India 2009" question group loaded
and you want to load "India 2010", the questions table will be corrupt as
described above
* is this issue related to MIFOS-4078?
