Question Groups not appearing as part of Create Loan workflow

Applied a PPI survey to everyone possible workflow. Survey doesn't show up
during Create Loan workflow.
Repro steps:
1. log in to http://ci.mifos.org:8116/mifos
2. Go to Admin, View Question Groups
3. note Create Loan groups:
Create Loan
* a2
* a4
* PPI Romania 2009
4. Create new loan for a client.
actual result: loan creation work flow completes with new loan, but no survey.
e.g. # 000100000000031 on this test server.
expected result: user sees page with the question groups mentioned above
during loan creation.
