When PPI Question Group is activated, user currently can edit the Question
Group title, which will invalidate the PPI data for the subsequent PPI
reporting.
Request the when PPI Question Group is loaded, the Question Group Title field
is disabled on the Edit Question Group page so the admin user is unable to
modify the title of the PPI question group.
