Questionnaire migration for 2.0 release used a java based way of migrating data
from custom_field tables into questions/question group tables.
This code is no longer needed and should be removed from code base and
apponly-services.xml config file.
As a result, this makes the 2.0 release a release that previous release must
upgrade to first before moving onto any other release.
Also, as the code is used in upgrade classes, these need to be removed and a
fixed schema for 2.0 release created - see MIFOS-4471, this should probably
happen first before removing code (task wise.)
