Remove old surveys in Elsie F

In F, we want to remove the old surveys code completely. E was the release to
migrate to Question Groups.
Under Admin, Manage Surveys section should be removed.
In entities where you can attach a survey, you should only be able to attach
question groups and view question groups now. We should assume that all old
surveys have been migrated to QG.
In those sections, it would be nice to replace any references to "Survey" with
"Question Group"
* So on the right side bar, instead of Surveys, it should say Question Groups
* Instead of Attach a Survey link, it should say Attach a Question Group
* Once you go into Attach, replace Survey with Question Group
