Unable to remove all fees when fee attached more than one time

While testing ability to remove fee prior to loan approval (MIFOS-2748), found
that I can get in a state where user can't remove all fees. Remove option is
available, but removal appears to do nothing.
Can view this behavior here:
test instance: http://birch.mifos.org:8120/mifos
loan: 000100000000437
try to remove fees 4054 test2:, 4054 test fee, or insurence fee.
Repro steps:
1. find loan that is pending approval.
2. add fee abc
3. add fee abc two or three more times.
4. remove fee abc
5. fee removed. repeat to try to remove all fees.
note the fees can't all be removed. Am only able to get down to one or two
fees remaining, but can't remove any others.
