Loan Creation does not validate that one-time loan fees can only be one

In F, we have changed Mifos so that you can only have once instance per
one-time fee for a loan. Currently in loan creation however you can still
attach multiple instances of the same time of fee.
Propose that when User hits Continue, validation is done to see if multiple
instances of the same fee had been attached and if so show the following error
message.
Multiple instances of the same one-time fee are not allowed.
