Offices are not properly assigned to parent offices on 'View offices' page

On 'View offices' page, offices are not properly assigned to parent offices.
See attached screenshot.
There are two regional offices 'RO dw1' but should be only one which contains
both 'BO 1' branch and 'br3', 'br4' brnaches.
Also when you click on details of 'br4' branch, then the Parent office for that
branch is 'Mifos HO' but should be 'RO dw1'.
