Node Reference field not working in several custom content types

In several custom content types, deployment projects and our normal page
content, a node reference field we're using for file uploads is no longer
working.
Previously one could click the plus sign and a window would pop up to upload a
file with description. Now when attempting to click the plus sign, you only
get a javascript:void(0) error in both Chrome and IE.
I didn't see anything that I could catch (in Chrome debugging) but I'm probably
not sure what to look for.
As far as I know, none of the node reference settings in the relationships tab
have changed.
