After you added missing file, I was able to upload and generate the report.
I compared the new version of report with old one and there are the following issues that should be fixed:
1. There is no report name.
2. Branch name, From, To labels are missing.
3. There is no header in the table with column names (GL-Code, Debit, Credit)
4. Sub-Totals label is missing
5. Generated by section is missing
I attached two screenshots on which you can see the differences (missing parts are selected with red box and related number).
