Description: Stack trace appear while adding new Question Group and wrong question is removed.

 To reproduce  stack trace (before any question was removed) :
1. Add Question Group
2. Add new Question 2 times
3. Try to change order
4. Stack trace appear

To reproduce wrong question removed:
1. Add new question group
2. Add 2 new questions
3. Remove first one
4. Add new question
5. Remove second one
Result: first one is removed.
Id assigned to question depend of the amount of questions, is not individual.
