Having looked at this I can the see the problem seem to occur when:
Through adding another table row onto dom, the scrollbar of the dialog becomes active as its out of visible space - For some reason, the click event listeners do not work on the delete buttons as a result (only for the buttons that when added cause scrollbar to be active/move)
I dont know how to fix this issue. It seems to be a jquery ui/jquery issue.
For charges its not so bad as its unlikey to at ten charges forcing it out of scroll pane but for accounting something will have to be done as delete wont work through UI.
