I'm moving this to the user's list.

A few question:

Is this fee added automatically for the product or something you select when 
creating the loan?
If its supposed to be added automatically, have you gone to the product 
definition and made sure its still there?
If automatic, are other automatic fees still attached to loan products?
Does the fee still exist under fees definitions?

This might help in narrowing down the issue.
