I totally forgot to mention that I can remove
/etc/mail/mimedefang-filter. same symptoms, no error messages.

I doubt this helps, but have you tried something like this in each of
the filter_ routines?
open F, ">> /tmp/mimedefang.log"; print F "FILTER_/*XXX*/ *stuff*\n";
close F;
that would at least tell you which filters worked and which failed?
