Can you confirm that the reverse-DNS lookup for this IP works and resolves to one of the names in the sendmail.cw ($=w) file?
If not, Sendmail resolves the IP address, gets a name it doesn' know about, assumes you want to relay to this address, and then refuses because you don't have that address (and/or the incoming relay address) in your permitted relay list.
