I'm sorry, I thought maybe something would jump out at me but nothing did. It looks like you are using network tests in SpamAssassin, since you've set $SALocalTestsOnly = 0. I would suggest temporarily disabling network tests in SA just to see if the issue goes away. Perhaps one of the RBL/Razor checks is timing out with certain types of messages?
