Good because I have a lot of connections that give a bad header( ie IP). So
I just but them in the access list and deny them. Then I thought if
mimedefang gets called before access list then this plan doesn't work and it
gets caught in the mimedefang filter which is a waste.

The short answer is "both".

Access.db gets checked multiple times, at multiple points during the SMTP
transaction, depending upon what SENDMAIL features you have enabled or
disabled (assuming you even have support enabled).

The milter also gets called multiple times by sendmail, during each
transaction.
