header BEEN_TAGGED ALL =~ /X-Spam-Flag: YES/
score BEEN_TAGGED 1000
and
header BEN_TAGGED X-Spam-Flag =~ /YES/
score 1000
I'm not sure about the cause, but have you tested the rule via
spamassassin -d --lint
to verify there are no syntax errors in your rule?
That helped me out with the first rules I tried to tackle.
