Lets go over this one more time, then if this looks right I guess I just need to go back over my whole setup starting at step 1.
As for directory permissions/ownership I have this in /var/spool:

(According to my passwd file, Sendmail ownership is thus):
smmsp:*:25:25::0:0:Sendmail Submission User:/var/spool/clientmqueue:/sbin/nologin
mailnull:*:26:26::0:0:Sendmail Default User:/var/spool/mqueue:/sbin/nologin
I have also tried mailnull as the group for the MIMEDefang directory.
In /usr/local/bin I have this for the files relating to mimedefang:

In /usr/local/etc I have a directory mimedefang as so:
drwxr-xr-x 2 root wheel 512 May 11 12:06 mimedefang
Within this directory I have this:
-rw-r--r-- 1 root wheel 14636 May 3 18:13 mimedefang-filter -rw-r--r-- 1 root wheel 14497 May 3 18:11 mimedefang-filter-original -rw-r--r-- 1 root wheel 14497 May 11 12:06 mimedefang-filter.example -rw-r--r-- 1 root wheel 2873 May 3 21:27 sa-mimedefang.cf -rw-r--r-- 1 root wheel 2872 May 11 12:06 sa-mimedefang.cf.example
This is the directory mimedefang is looking for it's filters in.
