There is no difference between rejecting in the milter and rejecting
in a ruleset -- you can't get at the headers until the message has been
transmitted anyway.
If you're not already using MIMEDefang, it's more efficient to do it
in sendmail.{mc,cf}. If you're already using MIMEDefang, it's more
maintainable (IMO) to do it in Perl.
