Have you ever hit performance problems caused by access table being to
big to be cached in memory?
[sendmail can do 30+ access lookups per one single SMTP session message]

I haven't had that issue but I would imagine the performance of an access table is still far better than a milter being involved. I agree with DFS' comment that sometimes you have to do it with a milter for flexibility, though.
