It could be that some other mail server added the SPAM: tag to the
subject, and the mail scored below the threshold on your system. You'd
need to add a bit of logging to your filter code to debug the problem.

all the [SPAM] emails that have made it into my inbox have a score of >10, and my spam threshold is 5. They're way over over the score, all of them, and yet mimedefang is letting them through....(?) it's still a mystery.
Here is a text copy of a couple of those spam emails including headers:

you can see in the headers the spam scores are up. Please remember that mimedefang is actually rejecting a lot of spam email on my server, but somehow letting others through. One thing I have noticed is that every [SPAM] email that gets to my inbox has:
'------=_NextPart_000_0007_01CAA19B.BF010B80--'
or something similar at the end.
