Are you certain you have the correct path(s)?

For me (who compiles Perl from a source tarball), the path where Perl gets 
installed is /usr/local/lib/perl5.

Also try putting your mimedefang-filter (or a symlink for it) in /usr/etc/mail. 
 Maybe an unexpected path got compiled in if you're using a pre-packaged 
version.
