Set your system to retain spool directories for a short period (-k option), 
send your test message again, and then manually scan the resulting INPUTMSG 
file or the Work/INPUTMBOX file with Clamd and see whether it detects anything. 
 These will be in /var/spool/MIMEDefang/mdefang-<MsgID>, e.g. 
/var/spool/MIMEDefang/mdefang-u14J88sD013402 

If it does, I'd be surprised - MIMEDefang acts on the return from the virus 
scanner, and the results from Clamav haven't changed format or anything, so the 
logical conclusion is that Clamd is no longer detecting the virus.  

If Clamd does find the test, then re-examine your filter, assuming it hasn't 
changed at the same time - if it has changed to bring it into line with 
something in 2.78, that would be my prime suspect.
