• rearrange the help cards; simple stuff like ^KS was missing!
• implement mathins (hack) and mathres for 3.7 and 4.4
• add ^QT for delbol, keep ^Q^? (^Q+backspace but we can’t do
this for ^H, as ^QH is already used), document only ^QT
• move isrch from ^[S to ^[T (conflict with ^[s for ^S)
• move open from ^[b (WTF?) to ^] matching joe
