first file merge, almost complete:
• RCSID (will be done upon merge into MAIN branch)
• various casts (likely most no longer necessary, although some,
like explicitly casting a sentinel nil to a pointer type, are new)
• use of OT_FMT in printf, casting the arguments
(this also will need dprintf stub consideration)
