Ah! Well I still think it would be useful to have documented somewhere.

Do I need to adjust my /etc/smtpd.conf anything, because I tried it and
couldn't get it to work. I just used the same pam configuration as I was
using for dovecot:

  # cat /etc/pam.d/smtpd
  auth    required pam_unix.so
  account required pam_unix.so
